The AFFIRMO research study will provide evidence about the use of an integrated care pathway in older adults with atrial fibrillation and multimorbidity (i.e., one or more other chronic conditions apart from atrial fibrillation). The AFFIRMO Study app (iABC) will facilitate and streamline the application of this integrated care pathway by providing patients with tips and suggestions regarding the management of their chronic conditions and lifestyle habits. If invited to participate in the AFFIRMO study, you can record your prescription information about use of oral anticoagulation, as well as viewing your adherence to oral anticoagulation medications. You can also enter observational data such as blood pressure and weight, and view graphs to monitor changes in these measurements. You will be able to receive weekly suggestions about the management of chronic conditions, questionnaires to monitor your symptoms, tips on maintaining your activities and you will also receive empowerment information, which is intended to help you remain as healthy as possible with your atrial fibrillation. The app was developed at the University of Manchester in close collaboration with people with atrial fibrillation, clinicians and researchers.
