このアプリケーションソフトは、在宅で医療的ケアを受けられている患者さんやご家族、介護をする方のために作成しました。災害時などに避難先でも治療や療養上の情報を提示することにより、円滑に支援が受けられるように使用するものです。
平時より情報を患者さんやご家族、介護者の手で入力し、情報の変更があればその都度更新するようにしてください。

本アプリは、以下の研究課題の成果として幅広く使用いただけます。
