## 健康家园(患者端) Description
随着“互联网+护理服务”实施方案的发布，广州创医元信息技术有限公司推出一款“健康家园”APP，依托互联网等信息技术，通过“线上申请、线下服务”的预约方式，为出院患者或罹患疾病且行动不便的特殊人群提供的护理服务。平台的提供主体均是实际确定取得《医疗机构执业许可证》并已具备家庭病床、巡诊等服务方式的实体医疗机构，它们可依托互联网信息技术平台，派出本机构注册护士提供“互联网+护理服务”，将护理服务从机构内延伸至社区、家庭。派出的注册护士同样有资质门槛——应当至少具备五年以上临床护理工作经验和护师以上技术职称，能够在全国护士电子注册系统中查询。护士可以利用业余时间来有偿帮助一些不便的老人、患者提供医院陪诊、上门护理服务。平台为用户提供高效的就医辅助服务，改善就诊体检，提高延续护理的质量，满足人民群众个性化健康需求。
该app为广东省人民医院实现延续护理配套使用，其中主要功能如下：
绑定：住院患者输入住院号从医院同步患者数据到app中。
订单：患者查看自己在医院预约的延续护理服务，并进行后续一系列护理操作。
