THIS APP WILL HELP INVITED MEMBERS: · Chat with a nurse through secure messaging · Create reminders to track medications and set appointments · Keep an eye on health and overall well-being progress · Get encouragement, checklists and daily tasks to meet goals · Access personalized education to improve their health Blue Cross Coordinated Care is an all-in-one, personalized support program that puts members at the center of their health care, and there’s no additional cost. If you’ve received an invitation to join this program, you will work with a registered nurse who is part of your care management team. The Blue Cross Coordinated Care app helps you virtually connect with your nurse and access a variety of helpful resources. BCBSM Coordinated Care works with Apple Health for certain care programs, such as those that provide a step count task. Seek a doctor’s advice before making any medical decisions. This app is designed for education and to provide support between provider visits.
