Gebaseerd op de principes van Waardegedreven Zorg, focussen wij ons op wat voor u als patiënt belangrijk is. De zorg die u krijgt moet voor u en uw gezondheid waardevol zijn. Daarom meet de DEARhealth app de waarde van uw zorg met een ziekte-specifieke Gezondheidsbeoordeling. Deze richt zich op drie aspecten van uw algemene gezondheid: ziektecontrole, kwaliteit van leven en dagelijkse productiviteit. U krijgt ook inzicht in het zorgpad die uw zorgverlener voor u heeft uitgestippeld. Dit betekent dat u weet welke vragenlijsten wanneer gepland zijn voor de komende maanden. Het doorlopend bijhouden van uw Gezondheidsbeoordeling zorgt ervoor dat u en uw zorgverlener kunnen reageren op veranderingen, bijv. uw zorgpad aanpassen. DEARhealth laat u: • Uw Gezondheidsbeoordeling invullen en uw scores over de tijd bekijken • Uw gepersonaliseerde zorgpad bekijken met aankomende vragenlijsten • Leren over uw ziekte met behulp van het programma ‘Mijn academie’ • Toegang krijgen tot extra Welzijn programma’s zoals Mijn menu en Mijn coach Om gebruik te maken van de DEARhealth app, moet u een account laten maken via uw zorgverlener.
