Docket® partners with public health heroes to make it easier than ever for you to access personal and family immunization records directly from Alaska Department of Health, Idaho Department of Health and Welfare, Minnesota Department of Health, New Jersey Department of Health, Utah Department of Health and Human Services, and Wyoming Department of Health. Track upcoming shots, review past immunizations, and share your official immunization reports.

Alaska, Idaho, New Jersey, Utah, and Wyoming residents can access a SMART® Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions. So easy!!!

Easy to find record.

Frustrating

Updated…now overly complicated. Just show me my card!

Great to Have!

I don’t really want to carry my physical COVID vaccination card around. If you get it lost or your bag gets stolen, you have no proof to enter most venues and other places that require proof. This app took practically no time to download and enter the required information. If you live in the states that currently have this app, get it! It’s free and it’s purpose is invaluable.

Out of State Immunizations

I love this app and it works very well. However while vacationing out of state I had to get a tetanus shot. How do I get my Docket updated?
