Tips:
To not ignore reminders: Download on an iPhone paired with an Apple Watch. Go to the watch app: Notifications > scroll down to Eye Care > make sure slider is green. Now you will have notifications on your wrist whenever you are. Then go back to the main Watch app: Sounds and Haptics > Haptic Alerts > click so slider is green. This is so you can feel the notifications on your wrist.
To have notifications only when you need to: Use focus modes and put this app in the allowed lists for any modes where you still want to hear reminders to look away.

Developer doesn’t listen to my concern
I have been asking the developer of this app to be able to change the timer to less than 20 minutes since my doctor said that I need to looks way from the screen every 10-15 minutes. I am able to change the timer to any amount of minutes past 20 minutes, why then am I not able to change the time to less than 20 minutes?
Stopped working properly
So I downloaded this for work. First off, the timer sounds even on silent and I did not appreciate that at work. Then it got worse. The app worked fine for a couple days and then it stopped showing me 20 seconds and just went with the 20 minutes. Then, when I tried to restart it, it would just keep counting down. Not happy. Found another app I like better.

I really like it. It does help my eyes. The only thing lacking is a sound indicating the end of 20 seconds of looking away. I thought I read that the paid version had it, but it doesn’t.
Kinda useless
Set it to go off during my work hours. It only goes off if I happen to be looking at my phone at that moment. Considering all my work is done on a computer, that doesn’t seem to be a reliable solution. If I’m on my phone in the evening, it continues to go off well after hours. Not sure what the point is.
Simple but functional
I sit at a computer all day long and get terrible headaches. My doctor said to look away every 20 min and this simple app is exactly what I needed to remind me to do so.
Waste of money
Don’t bother getting the regular version or the paid version. They both lack proper functionality. You are way better off just creating timers on your phone
I was getting daily headaches until I used this app
I do all of my work on screens, and after work, I am looking at screens. Around 3pm-4pm I would get headaches, everyday. I started using this app to look away for 20 seconds every 20 mins. Since this, no headaches! Very grateful and highly recommended. Why not 5 stars? The app goes off when I’m not at work and I can’t schedule it to be only M-F. To remedy this, I mute the app on the weekend.
