Disclaimer:
In the ADHD DISK, users record their own experienced symptoms and quality of life based on a scientifically developed Patient-Reported Outcome Measures Survey (PROM) and can then view their ratings. The DISK ratings enable users to record their symptoms and quality of life in one place and access the information when needed. For example: to facilitate a dialogue between the user, Health care professionals and loved ones. The data visualizations do not constitute a clinical assessment and are not intended to affect the treatment of the patient or their management in the clinic.
