An app to manage your care remotely and for connecting your clinical care team with your health and symptom information.
With PRIME, you may be asked to do any of the following:
- Log your vitals and output
- Answer symptom-specific surveys
- Take photos of your wound
- Receive alerts and reminders
- Review patient education materials
- Provide feedback to your provider
To use, your provider must first prescribe and enroll you in PRIME on CareConnect and you must have an active myUCLAhealth account.
