Technical Issue/User Error

I needed an app to help track and check off my step-by-step activities in a given task, ensuring all processes were completed. This app seemed(s) idea for that accomplishment for a more informal personal tracking method outside creating a paper form. The app downloaded quickly, while the methodology is well explained, the actual process in using is not. I would imagine this is due to a seemingly simple layout. Add your title, chose a color, initiate your first step, hit the plus-minus button to create the next step, repeat, and save. Any point I attempt to add the next step, my initial step disappears leaving me in an endless loop of restarts. I have tried shutting down the app, saving and returning, and even going into edit mode with no change.
