Deze toename van het aantal kanker survivors is het gevolg van betere behandelingen. Deze behandelingen van kanker zijn vaak complex. Na afronding van de behandeling hebben survivors kans op ontwikkeling van lange termijn toxiciteit en late effecten. De overgang van patiënten van hun oncologische zorg naar follow-up en nazorg (survivorship fase) confronteert survivors met een aantal uitdagingen: herstellen na een behandeling van kanker en het optimaal houden van hun gezondheid en algemeen welzijn. Voor survivors van kanker en hun zorgverleners is het gewenst en handig om de beschikking te hebben over een relevante samenvatting van de gegeven behandeling en een nazorgplan, ook wel survivorship care plan (SCP) genoemd. Dit SCP helpt de survivors en hun zorgverleners om goed door de follow-up fase te navigeren. In het begin richt de follow-up zich vooral op het vroegtijdig vaststellen van de terugkeer van de ziekte. Later in de follow-up zal de focus meer liggen op de late effecten en het behoud van gezondheid. Voor survivors is het van belang om zelf de regie te krijgen over hun eigen follow-up. Dit kan samen met de oncoloog en de huisarts en wordt dan shared care genoemd. De Survivor app is ontwikkeld om deze vorm van survivorship care mogelijk te maken. De Survivor app is ontwikkeld voor patiënten die zojuist hun behandeling voor kanker hebben voltooid en beginnen met de follow-up met eigen SCP. De app biedt informatie over follow-up en herstel na een behandeling van kanker. U kunt interactief aantekeningen maken, vragen noteren voor uw dokter en de antwoorden opnemen. Voor zaadbalkanker survivors heeft de Survivor app als extra mogelijkheid het inscannen van een SCP. Het persoonlijke SCP wordt ingelezen met een QR-code. Deze QR-code wordt gegenereerd door de behandelend arts op de polikliniek en bevat belangrijke afspraken en adviezen voor de follow-up. Na het inscannen kunnen de gemaakte afspraken worden toegevoegd. Deze worden vervolgens gesynchroniseerd met de kalender van de iPhone.
