本計劃的旨在為首發急性心力衰竭患者出院後作動態遠程監護，以改善和優 化急性心力衰竭患者的指引導向藥物治療。我們相信是次研究的成果將有助 於評估遠程監測用於急性心力衰竭患者的可行性，希望能在未來為患有急性 心力衰竭患者提供更多院外支援，亦能在心力衰竭惡化之前更早地進行評估 和管理。
