Akcja Super Meat Boy Forever rozgrywa się kilka lat po wydarzeniach, które miały miejsce w grze Super Meat Boy. Od kilku lat Meat Boy i Bandage Girl wiodą szczęśliwe życie wolne od Dr. Fetusa, wychowując cudowną córeczkę Nugget, która jest prawdziwym uosobieniem radości i oczkiem w głowie rodziców. Pewnego dnia, gdy urządzają piknik, Dr. Fetus zastawia na nich pułapkę. Otrzymawszy cios łopatą, Meat Boy i Bandage Girl padają bez świadomości, a złoczyńca porywa Nugget! Gdy nasi bohaterowie dochodzą do siebie i okazuje się, że Nugget zniknęła, nie mają wątpliwości, kto jest sprawcą. Postanawiają, że nie spoczną, póki nie odzyskają Nugget i nie udzielą Dr. Fetusowi bardzo ważnej lekcji. Takiej, której można udzielić wyłącznie za pomocą ciosów pięści i kopniaków.
