## UH MyChart Description
University Hospitals (UH) MyChart is a personalized health app that helps you conveniently manage care for yourself and your family members. With the UH MyChart app you can:
• Schedule and manage appointments, including in-person visits and video visits.
• Review test results, medications, immunization history and other health information.
• Communicate with your primary care team.
• View and pay your medical bills.
• Connect your account to Apple Health to pull health-related data from your personal devices right into MyChart.
• View your After Visit Summary® for past visits, along with any clinical notes your provider has recorded and shared with you.
• Connect your accounts from other healthcare organizations so you can see all your health information in one place, even if you've been seen at multiple healthcare organizations.
• Securely share your medical record from anywhere with anyone who has Internet access.
• Receive push notifications when new information is available in MyChart. You can check whether push notifications are enabled under the Account Settings within the app.
To access UH MyChart, you must create an account. Access is free and secure.

Using My Chart is the best medical communicator. It avoids having to be put on  hold at the doctors office and then waiting for a call back. Often, you need to call several times. With My Chart, you can get right to the point and I have always received an answer in a timely manner. My test results come back as soon as available.  I can make an appointment with ease at a location of my preference. My chart is ACES!!!!

Dr Pamuk said to contact him through My Chart after my October 30 appointment. I can not find a way to contact him. I love having visit summaries and text results through My Chsrt, but how do I contact a doctor when he asks me to???  Please help!

Love the ease and organization of everything in one place. But it’s hard to find your “tasks”, as there isn’t a separate button for your blood or work orders.

It took me a while to figure out that the links to My Chart from email do not work; I’m notified but must access through the app. Now I’m trying to figure out how to get my daughter on as Proxy. The directions could use more detail. Does my proxy need to download the app? The link in the invitation didn’t work. I like that there are videos to help but again, directions lack specificity. As long as you know what you are doing it’s great but the minute a new process is available it takes resources and time to figure it out. Not everybody is tech savvy and those that need  a proxy may be elderly and not tech oriented.

Every single time I have an appointment or lab work or order medication there is a problem they don’t do it on time or they don’t do it at all. They don’t order all the bloodwork and I live in Conneaut so it’s real hard to get to mentor for their mistakes, I never had this problem at any other doctor that I’ve been to, but they’ve all closed down or moved or retired by this point. It’s very disheartening. When Doctor time comes around or they need some thing from her, I dread it because I know it’s going to be some kind of a fight of some sort to get what need right now. I don’t have my blood work completed because I don’t think she ordered it all. It’s just unbelievable. I wish I could just go in peace

Very difficult to have updated and accurate medicine listing on MyChart. This is partially a problem with the UH Doctors and staff not using and updating the system properly. This has caused issues during urgent care or emergency room visits. Closely linked to this is an inability to request renewal of prescriptions through MyChart in a simple streamlined manner. This ties in with UH still being plagued by a horrible telephone communications system, which frustrates patients, family and UH staff.
