此社會資本計劃由社區投資共享基金資助，香港中文大學醫學院那打素護理學院負責推行。
本計劃運用「社、校、醫」跨界別協作模式，以健康為切入點，希望透過一系列社區健康教育活動，提升水泉澳邨居民的健康意識。本計劃亦積極推動區內居民組織義工團隊，攜手建立鄰里互助網絡，把水泉澳邨打造為居家安老的社區。
