In the diagnosis , it should be added one more option as ‘others’ to write description. It’s very difficult to find the exact diagnosis from the drop down options .

Claim form out of date

Firstly I was not able to submit my claim on line due to your claim form template undergoing some upgrade and it would not accept uploaded pdf docs. So I had to file on the mobile app. Again the form is outdated: neither the panel clinic nor the type of clinic (panel specialist) were on the drop down lists of options to select, so I had to choose the nearest option then write in the comments box to explain that your form is inaccurate. All this makes filing a claim tedious and difficult. Please do better.

The doctor’s description of the ailment may not coincide exactly with the diagnosis list available in the app and the user is forced to choose one of those that appear to be. User is baffled by all the various medical terms used
