该作以公元2世纪到3世纪中国的魏、蜀、吴三国时代为背景，展开战场地图，玩家需要在一个接一个出现的战场上达成规定的胜利条件。
同英杰传系列以往的作品一样，《曹操传》前半部分较忠于历史，比如镇压黄巾军、群雄讨伐董卓、灭吕布、剿袁术、战官渡、鏖兵赤壁、争夺汉中等。后半部则可根据玩家在游戏中所选择的选项，改变曹操的结局。游戏中有两条不同的路线：一是按历史正统路线，等到刘备兵败后乘虚而进军蜀吴，最终统一全国；另一个是幻想神话路线，孔明受魔王控制，暗算刘备、联东吴战曹操，最后曹操获胜，统一全国。在神话结局中，玩家可以得到名震三国的猛将——关羽。其实再进行严格区分的话，在正统结局中还能再区分出“统一——称帝”和“统一——不称帝”两种。
在游戏中，曹操的活动涵盖了汉末和三国初建时期。所有的著名人物都将登场：董卓、袁绍、袁术、吕布、刘备三兄弟、诸葛亮、赵云、孙坚、孙策、孙权等在曹操称霸全国的道路上与之一决雌雄的人物。而曹操手下也是人才济济：夏侯惇、夏侯渊兄弟、荀彧、荀攸叔侄、司马懿、郭嘉、许褚、典韦等。
