完全看不到每行是要填寫什麼，底色淺色，每行說明填寫資料又用淺灰定，淺到看不到，連有幾多行要填寫都看不到，又沒有其他說明文件可看哪行是填什麼，一和二行完全不知道哪行是中文名哪行是英文名，第三行是估應該是電話號碼，第四行身份證
