「生活再動（護老者版）：智能健康管理應用程式」由香港賽馬會慈善信托基金捐助，基督教信義會社會服務部於2018年開始推行的「生活再動計劃」項目之一。本計劃結合科技、醫護團隊和護老者的參與，為長者重整健康生活習慣及進行康復訓練。
長者透過下載及使用「生活再動（長者版）：智能健康管理應用程式」，便可安坐家中接受合適的復康運動訓練、大腦認知訓練、遙距健康監測、社交及網上活動、娛樂及健康資訊、健康生活日程等服務，讓長者的生活再動起來，重拾魄力及自信！
而照顧者或職員更可透過「生活再動（護老者版）：智能健康管理應用程式」實時了解長者版所紀錄的身體狀況，並了解長者參與當中訓練的情況及表現，亦可以使用程式中的通訊工具與長者交談。
