The Pediatric to Adult Care Transition (PACT) mobile application is designed to assist youth who are making the transition from pediatric care to adult care. This app helps ensure that they with know the necessary knowledge and information to continue to receive the care they need. This app was made possible by the efforts of Kimberly Campbell, RN, and Olcay Y. Jones, MD, PhD, in the Subspecialty Service of the Department of Pediatrics at Walter Reed National Military Medical Center (WRNMMC) in collaboration with the Office of Command Communications at WRNMMC and the Defense Health Agency’s Web & Mobile Technology Programs Group at Joint Base Lewis-McChord.
