Essentially just takes you through the Rockwood Clinical Frailty Score - you’re better off just have the aid memoir diagram of the stages in your phones photos - just find the diagram on google.

A much awaited tool, this should be used in both primary and secondary care. I have already adopted it and shared with my colleagues, both in primary and secondary care.
