i cant sign up to the last step also

I use to be able to see my health records. I want to keep track of my well being. But now it wont show anything. In fact i cant find the clinic that i went. ???? Developer do something

Tried new sign up but couldn’t complete at the last step. If sign up can’t be made, what for this app is created???

Dear dev team please help to fix bug for new user registration. Unable to complete sign up process and allow registration using email address as well. Didn’t catch any fields asking for email ID as secondary registration method. Thanks.
