Highly customisable personal forms

2009-03-01, v1.6: I love this app. It's practically all you need to store and catalogue your personal data in a secure manner. For example, I wanted to save a list of all the medications that I have to take, but didn't want to keep saving it in Notes, or email. This app allows you to create or customise an existing template with your own specific needs. So now I have a form that allows me to easily input the doctor's name, date of visit, name of prescription, what its for, and dosage intake. It's very easy to use and customise. In one word, it's brilliant. And the developer responds quickly to emails and even forum requests, which game me the confidence I needed to have continued support throughout the use of this app. Very highly recommended!
